1. The Water Service Application fee must be fully paid before a water meter is installed in the customer's listed connection address (residence or business). Additional Residential Connection Pipe (RCP) must be purchased from CWD. As per Board Resolution 2014-005

The Basic Service Connection based on the Classification of Pipes and Meters:

½ “ = P 3,150.00

¾ “ = P 4,083.00

1 “ = P 7,401.00

2. Owner of the said water meter still belong to the water district.

3. The customer shall take the water service on a month to month basis automatically renewing, unless otherwise terminated by the Water District shall furnish water service to the Customer at his installation located at his given address as shown in his record at the stipulated and under the terms and conditions as forth in the metered rate schedule. It is understood that the Water District reserves the right to adjust the water rates schedule even without prior notice to the customer.

4. The customer binds himself to pay at the office of the Water District, for water service consumed within fifteen (15) days from the reading date. The Customer binds himself to pay an additional sum of ten (10%) of his current bill as penalty in case the payment is received after the due date.

Example:

Reading Date - January 1, 2015

Due Date - January 15, 2015

10%Penalty - January 16, 2015

Ten (10) days grace period - January 16-25, 2015

In case the Due Date falls on a weekend or holiday, the date will be moved on the next business day.

5. The Water District shall not be held responsible for any interruption of the service or damage to property due to causes beyond its control.

6. The Customers shall allow employees and representatives of the Water District to enter the premises of the customer's properties without being liable of trespassing for the purposes of disconnecting services if no payment of bills were made; inspecting installing, reading, removing, testing replacing or otherwise disposing of its apparatus and property and/or removing the Water District's properties upon the termination of this Agreement for any cause.

7. The Customer hereby agrees to pay all his unpaid accounts and the reconnection fee of 150.00 prior to restoration of water services in case the service has been previously disconnected on account of delinquency of payment and/or violation of this agreement.

8. The Water District reserves the right to discontinue service in case the Customer is in arrears in the payment of bill for one (1) month and Ten (10) days grace period or the failure to pay the adjusted bills in those cases where the meter stopped or failed to register the correct amount of water consumed or to prevent fraud upon the Water District.

9. The customers shall pay the cost of necessary repairs or replacement in the event of loss or damage to property of the Water District through misuse or negligence on the part of the Customer, his employees or any member of the household, and he will be held responsible for tampering, interfering with or breaking of seals of meters or other equipment of the Water District installed and will be held liable for the same in accordance with the law and likewise the customer shall be held liable on the loss of the water meter installed in their premises due to any cause and shall pay its cost to the Water District.

10. The customer shall maintain the installation in proper condition and hereby guarantees that no tapping whatsoever shall be made on the service pipes and no sub-connection shall be installed without prior written authority by the Water District. 11. Service pipes and their accessories installed within the road right-of-way are considered automatically donated to and become property of the Water District.

12. The customer hereby binds himself to follow all existing rules and regulations set by the District as well as those that from time to time be promulgated and implemented by the Water District.

13. In case the customer decides that his services be discontinued or otherwise terminated, he shall inform the Water District in writing and which shall cause the disconnection of the water service. Optionally, the customer can request for a VTL (Voluntary and Temporary Locking). The current fee for this is Php 50.00. No disconnection shall take place unless the customer pays all unpaid bills and the request is approved and authorized by the Water District. 13. It is prohibited for the Consumer to profit from selling water that came from the Water District. Penalty is outright permanent disconnection .

14. “ By entering this contract I hereby undertake that I have no other obligations between a Water District this is a direct definition and I am applying for this water connection without any intention to be denied other liability.” ( Excerpts from the Nov. 20, 2014 Minutes of the meeting )

16. Senior Citizen Act Article 12 Section 1. Five (5%) Discount – The grant of a minimum of five percent (5%) discount relative to the monthly utilization of water and electricity by households with senior citizens; Provided, that the individual meters for the foregoing utilities are registered in the name of the senior citizen residing therein, provided, further, that the monthly consumption does not exceed one hundred kilowatt hours (100kwh) of electricity and thirty cubic meters (30m!) of water. Provided , furthermore, that the privilege is granted per household regardless of the number of senior citizens residing herein.

To avail of the discount under this Section, the senior citizen shall:

1. Apply for the discount personally or thru a representative. There shall be annual renewal of application to the utility provider.

2. Submit requirements.

3. Proof of billing. Meter registration should be in the name of the senior citizen for a period of one year.
