| 2. Submit a copy of Community Tax Certificate (Sedula) and Valid I.D. of any of the following: - TIN - Driver's License - Passport - GSIS/SSS card - Voter's ID |

| 4. For water service applicants who are not the owners of the house and lot where the water service connection will be installed and or tenant of an apartment, he/she is required to submit the following: Letter of authorization from the apartment/land owner Homeowner's Authorization Permit (HAP) |

| 5. After submitting the application, the service application fee shall be paid at the cashier. | CASHIER | 1 MINUTE | |
| 6. After fees have been paid, wait for an orientation from the CWD Staff with regards to water services, fees and water usage use. | COMMERCIAL SECTION | 1 MINUTE | |
| 7. Before the meter will be installed, an inspection will be conducted as to where the location of the house of the applicant and as to where's the nearest source or mainline or stubout the applicant's service line will be tapped. | INSPECTOR | 3-5 MINUTES | |
| 8. Applicants cannot lay out their pipes unless and until given a go signal/approval by the area inspector. | INSPECTOR | 1 MINUTE | |
| 9. Installation of the water meter can be done not more than seven (7) working days after payment of water service installation fee.
